Through the On Premise™ program, a trained professional—called an On Premise Extender (OPE)—seamlessly integrates into existing organization workflows to become a member of the care team. Armed with reliable and current information and analytics, this individual ensures any indicated care is provided to a specific group of Members of Interest (MOIs), verifies that patients are engaged to improve self management and checks that documentation and coding are accurate and complete. The OPE works directly with MOIs to optimize care delivery and ensure that all scheduled appointments are kept.

A full-time OPE may support a single or multiple health plans or a provider group, engaging in year-round data collection and providing focused member attention. Having this dedicated resource at your disposable helps your organization close any care gaps without having to pull existing staff and clinicians off task.
