For example, they offer three-way calling assistance to schedule patient appointments with providers. Once an appointment is made, advocates fax providers and follow up to make sure scheduled appointments are kept.

HAP™ professionals can also personally deliver your organization’s health advocacy message, such as a welcome for new members or tips for chronic care management. They convey information in a simple and clear way and verify a patient understands the information provided.
