Health Advocacy Programs (HAP™)
Patients are more likely to engage in their care if they understand treatment options and appreciate their role in care management. Unfortunately, many patients don’t have a clear understanding of their care needs and may have challenges navigating the complex healthcare arena.
The Health Advocacy Programs (HAP™) rely on specifically trained advocates to reach out to patients and partner with them to achieve care goals. Skilled in high-touch communication, these advocates foster trust with patients and engage them in care management activities. Whether it’s making an appointment with a primary care physician or coordinating a home assessment for the home-bound member, health advocates facilitate the process. For example, they offer three-way calling assistance to schedule patient appointments with providers. Once an appointment is made, advocates fax providers and follow up to make sure scheduled appointments are kept.
HAP™ professionals can also personally deliver your organization’s health advocacy message, such as a welcome for new members or tips for chronic care management. They convey information in a simple and clear way and verify a patient understands the information provided.
The Health Advocacy Programs bring trained advocates directly to the patient to address any care disparities and support patient compliance with targeted care management interventions.
